After you fill out the interest form, a member of our team will reach out to coordinate a time for you to come into our office. Once you get here, we will talk with you, ask questions, and learn more about your experience at CHDC. You will have the chance to tell your story, thank your providers, and inspire others!

Right now, we are seeking patients who are willing to tell their story while we take a video recording. These stories will be shared, with your permission, on our website, social media, waiting room monitors, and other various marketing materials. If you would prefer not to be filmed but still want to participate, please fill out the form anyway! We may still reach out for a written testimonial.

All you need is yourself! When you come in for your recording, we will have some questions ready for you to get the conversation going. There is no pressure to have anything rehearsed or written down ahead of time!
