At CHDC, we provide high quality care to all patients, regardless of their health insurance status or ability to pay. As a Federally Qualified Health Center (FQHC), our mission is to ensure that cost is never a barrier to getting the health services you need to stay well and live a healthier life.

What health insurance plans are accepted?
We accept most major Commercial, Medicare and Medicare supplement plans, Fee for Service Medicaid and Managed Care Medicaid plans, and can provide a health care discount for eligible uninsured and underinsured patients. Click on the dropdowns below to see which plans we accept at CHDC.

How do I apply for health insurance?
If you don't currently have health insurance, our Case Management team can help! For current CHDC medical patients, we are able to assist in applying for Medicaid (Medical Assistance) or CHIP, enrolling in a Marketplace insurance plan through Pennie, and help with understanding your coverage options. Our Case Management team provides services at our Shoppes at Coventry and Barto locations.
I don't have health insurance right now. Can I still come to CHDC?
Here at CHDC, we accept patients regardless of insurance status. If you are uninsured or underinsured, please give us a call at 610-326-9460 to speak with a member of our Patient Services team about establishing CHDC as your healthcare home.
What is the Healthcare Discount program?
All patients are encouraged to apply for our Healthcare Discount program. Our Sliding Scale Fee adjusts the cost of your healthcare bill based on household income and family size, following federal poverty guidelines. Even if you have insurance, you may still qualify for a discount! Learn more about our Healthcare Discount Program.
For qualified patients receiving a 100% healthcare discount, the nominal fee for a medical/behavioral health/vision visit is $25, $40 for a dental visit and $10 for a prescription! CHDC can offer the health care discount because we receive annual funding from our Federal partner, Health Resources and Services Administration (HRSA) since 2012 as a designated Federally Qualified Health Center.
