For Established CHDC Patients!

Located in Pottstown, our 700 Heritage Drive office offers Family Medicine and Express Care on the upper level and Emergency Dental services on the lower level.

Express Care is available for established CHDC patients needing acute or sick visits. Walk-ins welcome, with final walk-ins accepted 30 minutes prior to closing.

This location provides high-quality, compassionate care to our patients throughout northern Chester, western Montgomery, and southeastern Berks counties.

Sick and Acute Care for All Ages: Established CHDC patients of all ages can see one of our compassionate family medicine providers and be treated for, but not limited to:

• Allergies
• Body aches/pain
• Cold and flu symptoms
• ear infections
• employer health assessments (walk-in physicals and TB testing)
• Falls (minor)
• Fever
• Laceration (cuts)
• Nausea, vomiting, diarrhea
• Rash
• Sinus infections
• Sore throat
• Sprains
• Upper respiratory infections
• Urinary tract infections (UTI)

Emergency Dental Services: Express Dental services are available for established dental patients who are experiencing:

• Simple extractions
• Tooth or mouth pain

We believe everyone deserves access to quality healthcare, regardless of insurance status or ability to pay. That's why we offer affordable care options and a Sliding Scale Fee program based on income. All patients are encouraged to apply for a Healthcare Discount whether you currently have insurance or have gaps in coverage. We accept most major commercial insurance plans along with Medicare and Medicaid. CHDC also offers on-site language translation services and bilingual staff.
