Recent changes to Medicaid eligibility across the U.S. have put many individuals at risk of losing their coverage. Flexible Payment Options

We accept most major insurances, including all PA Medicaid HMO's, DHS Medicaid, Medicare, and commercial plans. If you have no insurance, but still wish to visit a CHDC, we can help! We offer a Healthcare Discount Program that is based on your family size and household income. Patients in our service area throughout Berks, Chester, and Montgomery counties who are eligible can receive discounts of up to 25%, 40%, 55%, 70% or 100% for services received at CHDC. Supplemental Nutrition Assistance Program (SNAP) Notice

With the recent incidents of SNAP benefits thefts, the PA Department of Human Services is suggesting recipients change their EBT card pins ahead of their monthly benefit distribution. The theft of benefits rarely happens right away but thieves wait until the monthly distribution occurs to steal benefits. Read here for more tips.

We encourage SNAP recipients to change their EBT PIN at least monthly and right before their payment date to prevent theft.

How to Change Your EBT Card PIN:

By Phone: 1-888-EBT-PENN (1-888-328-7366), open 24 hours a day, seven days a week.
Online: Through DHS’ EBT contractor, Conduent, via their Connect EBT website.
Via Mobile Apps: Either the ConnectEBT app or myCOMPASS PA mobile app

FREE Annual Medicare Wellness Visit

Have you had your FREE annual Medicare Wellness Visit (MWV)? We waive your deductible, you save $200! Visit CHDC for your Medicare Annual Wellness Visit and meet with our Case Management Team to see if you qualify for additional savings!

Qualified participants may be eligible for Medicare Extra Help program to save even more on prescriptions.
Qualified participants may be eligible for Medicare Part B Premium savings of up to $174.70 per month!

When to schedule? You can schedule an Annual Wellness Visit 1 calendar year since your last Annual Wellness Visit.

How much does it cost? With Medicare Part B, you can get a wellness visit at CHDC once a year at no cost to you.

How do I schedule? To schedule, give us a call at 610-326-9460 and ask to schedule a Medicare Annual Wellness Visit.
