Case Management Services
Our on-site Case Manager provides comprehensive care coordination to patients with the goal of improving an individual’s health and social well-being. We are Community Partners with the PA Department of Public Welfare. Our Case Manager utilizes the web-based program Compass, to determine eligibility and assist patients in applying for state programs such as Medicaid, Select Plan for Woman, SNAP, Cash Assistance and the Pennsylvania Low Income Home Energy Assistance Program (PALIHEAP).
Case Management will aid the uninsured and underinsured in obtaining free or low-cost medication through a variety of web-based Patient Assistance Programs. Our case managers will also track and re-order prescriptions for our patients, while also assisting with Social Security’s Medicare and Medicare Part D programs.
Due to the extensive collaborative efforts within our community, case management is also able to provide referrals to appropriate human service agencies according to the needs of our patients.
Medicare Open Enrollment 2024
Medicare plans change from year to year, just as your healthcare needs do. "Open Enrollment" for Medicare changes begins October 15th and runs through December 7th. This is when beneficiaries can join, switch, or drop Medicare Advantage, or Prescription Drug Plans. Medicare coverage changes go into effect January 1st, 2024. This is the primary time when people with Medicare can choose a plan, but there are other times during the year that may be called “open enrollment” or “special enrollment” periods when a person can enroll in a plan.
CHDC’s Case Management team will help Medicare recipients navigate the Medicare registration process and understand the Medicare buy‐in program which helps eligible individuals pay for certain out‐of‐pocket healthcare costs. Individuals do not need to be a CHDC patient to take advantage of this free service. Seniors with limited income and resources may qualify to have their Medicare Part B premium paid for, saving thousands of dollars by receiving maximum benefits from the Medicare program.
Medicare beneficiaries are encouraged to call and make an appointment with our Case Management team. Our team may be able to assist in applying for "Extra Help", which lowers the cost of Medicare Part D coverage and prescriptions for those who qualify.
