9/7/21: Important Vaccination Updates

In addition to offering the first and second dose of the COVID-19 vaccines, CHDC will also be administering third doses to those who qualify. Individuals who are moderately or severely immunocompromised can get the third dose, at least 28 days after the second dose. For everyone else, CDC recommends getting the third dose 8 months after the second dose. However, this decision is not final and we will update when final guidance is given.

Pfizer vaccines are available for individuals ages 12+, and Moderna vaccines are available for individuals 18+. In order to receive the third dose of the vaccine, you must bring your COVID-19 vaccination card as this will be used to determine your 3^rd dose date. Please note you may not receive your 3^rd dose if you forget your vaccine card. Please schedule your appointment using the email or phone number below.
