CHDC's Center of Excellence

What is a Center of Excellence (COE)?

The Center of Excellence earned its designation from the Department of Human Services to be utilized by anyone with a current or previous diagnosis of opioid use disorder. The COE consists of Community Based Care Managers and a Certified Recovery Specialist (CRS).

What does a Center of Excellence (COE) do?

We use both internal and external resources and referrals to provide the best addiction treatment services. Each person gets their own Care Manager, who coordinates the many aspects of a recovery program.

Internally, we refer to medication-assisted treatment, medical services, and referrals to behavioral health counseling. Externally, we partner with organizations such as 12-Step Meetings, Sober Living opportunities, and more to provide care outside of our office.

We are here for you because recovery is possible.

Medication-Assisted Treatment

At CHDC, we have developed a strong Medication-Assisted Treatment (MAT) program that includes a board-certified addiction specialist, and a licensed professional medical team who specialize in treatment for opioid use disorder and alcoholism.

Our Patients Say It Best!

"I've been going here for years, and the whole nursing staff is great. There's free transportation and drivers, (Michael) go out of their way to help! The medical assistants, (Wanda), and let's not forget our CRS, (Deliverance), and Dr Netaji, and Dr Kristen are the greatest! Last, but not least, Brittany!! Brittany has also helped with so much in these past years! I definitely recommend Community Health and Dental Care!"

- Patient K.C.

"I have nothing but GREAT things to say about the MAT dept. They listen to what I have to say, and they always go above and beyond! Kirsten is the best provider I ever had! Can't say enough good things about her!! Deliverance is my life saver! She is always helping me make/change appointments and anything else I need. They truly do care."

- Patient B.S.

"This program has been a blessing for me. I highly recommend you make them a part of your family because that's how they treat me there. They are reliable, always available, and they actually care."

- Patient J.M.T.
