HANDS OF GIVING
CAMPAIGN

Community Health & Dental Care, Inc.
Annual Hands of Giving Campaign

The Hands of Giving Campaign is a new campaign established for the employees and any other donor who wishes to contribute monies directly for our patients of the health center, CHDC community event or any agreed upon charity within the community.

HOW IT WORKS

Employees who wish to contribute $3.00 per payroll will be entitled to dress down in jeans with a CHDC logo shirt every Friday while working at the health center. The $3.00 will be deducted automatically from each employee’s paycheck with the written consent of willing employees. Please see the hands of giving employee payroll deduction authorization form to sign up. Employees who dress down must wear the Hands of Giving magnetic pin so patients and community members understand dressing down is for a cause and in addition it signifies the employee is signed up for this campaign. Human Resources will provide the pin and the payroll deduction form. If the pin is lost, the replacement fee will be paid by the employee.

Employees are also welcome to make an annual donation of any amount but must be at least $78 to qualify to dress down. Please remember this is a charitable contribution and due to CHDC being a 501c(3) corporation, the annual donation may be deductible according to IRS rules and regulations.

Non-employees, volunteers, and the members of the community will be asked each year if they are willing to contribute a donation for an annual appeal and/or fundraising campaign.

The monies collected for this campaign will be accumulated and accounted for under the restricted patient fund account. Some examples of what the campaign can do for our patients and the community are but not limited to:

HOLIDAY VOUCHER PROGRAM

A preset amount of food vouchers can be distributed to the neediest patients within the health center which will allow them to go to a local grocery store and turn the voucher in and receive a full turkey dinner for themselves and their families.

Charity Event

In the past the health center has worked together to collect donations for specific events such as the Relay for Life and with the money collected from the Hands of Giving campaign a larger donation can be made possible.

FREE PUBLIC SUPPER

Money received from this campaign can be used to support free public suppers for the health center to reach out to the community and provide information about the services at the health center.

Back To School Supplies

A voucher system could be established for children at the health center who need school supplies using the Hands of Giving funds.

Emergency Funds

If an employee identifies a patient in need of food or any other life-sustaining supply which they can not afford, funds can be set aside to assist our patients.

These are just a few ideas of how the money collected can help our patients and the community we live and

work in. The ultimate goal is to continue to grow this campaign within the community but we would like to start it off with the employees and in return the employees are allowed to dress comfortably every Friday and feel great about directly giving to our patients and the community. Feel free to sign up today and join us with this wonderful giving campaign! Thank you for your support!
