Our full-service health center provides comprehensive primary medical care for both adults and children, including well visits, school and sports physicals, and immunizations. Patients can also access family dental services, such as routine cleanings, exams, fillings, and extractions. Downstairs, our physical therapy team offers rehabilitation and pain management for injuries or chronic conditions, while our behavioral health staff provides compassionate counseling and mental health support. To improve access for our community, we also offer free transportation services for patients who need help getting to their appointments. Whether you're managing a chronic condition, scheduling your child’s checkup, or looking for a new dental home, our Barto office is here with personalized, affordable care that meets the needs of your whole family.
Our Barto office is designed to make healthcare more accessible to the patients we serve. We believe everyone deserves access to quality healthcare, regardless of insurance status or ability to pay. That's why we offer affordable care options and a sliding scale fee program based on income. All patients are encouraged to apply for a Healthcare Discount whether you currently have insurance or have gaps in coverage. We accept most major commercial insurance plans along with Medicare and Medicaid. CHDC also offers on-site language translation services and bilingual staff.
