Integrated Behavioral Health Services

Family, Stress, and Everything Else

In our Total Care Program, we believe improving your health means more than checking blood pressure and heart rates. That's why behavioral health matters so much to us. Stress from life's struggles can manifest itself in a variety different ways and health issues. When life gets difficult to handle, CHDC provides behavioral health support

We offer a fully integrated behavioral health and medical program. Equipped with a team of behavioral health consultants, we work with your primary care provider to offer the best quality treatment available. At CHDC we help you through your problems. We walk the path with you, in hopes to give you a better quality of life.

Practice Self Care. "Self-care and coping skills can reduce stress and leave you with more energy to help care for others." Rebecca Green, LPC Behavioral Health Consultant.

Depression & Anxiety

Depression and anxiety make life difficult, just like any other chronic illness. Like diabetes patients need to maintain their blood sugar, people with depression and anxiety need to maintain their mental health.

We screen for these symptoms at all our patient visits, with the goal of maximizing your quality of life. Our staff is qualified to help provide you resources and tools to fight mental illness. From support groups to medication, our integrated behavioral health specialists treat patients on a person-by-person basis.

Our Behavioral Health team works with our Case Management team to connect you with any resource you may need. For example, we may refer you to one of our partners for additional support.

Our Patients Say It Best!

"I've had nothing but good experiences here. I am glad they work on a sliding scale also, to help everyone get the care they need. The facilities are clean, modern, and bright, and each provider I have had has been knowledgeable, kind, and professional."

- Patient D.M

"They were so wonderful and understanding!! I wouldn't go anywhere else. Very happy with the whole experience 🙂🙂🙂"

- Patient L.F.

"This program has been a blessing for me. I highly recommend you make them a part of your family because that's how they treat me there. They are reliable, always available, and they actually care."

- Patient J.T.

"I have had more than a fair share of medical care in my 64 years, between myself and my family. Hands down. No question. The care I have received here is the best I have ever had in all those years! They care for and treat the whole person mind, body and spirit. The staff at this location, from the moment you walk through the doors and hit reception clear through your entire experience, whether you are there for dental, medical, vision care, or to get lab work you couldn’t be in more competent and compassionate hands. Appointments are on time. Never do you leave without your questions answered. You are not just a problem or illness, you are treated and cared for as a whole person not just a list of symptoms or an illness... There are not enough Stars to rate community health and dental high enough."

- Patient S.W.
