We offer a fully integrated behavioral health and medical program. Equipped with a team of behavioral health consultants, we work with your primary care provider to offer the best quality treatment available. At CHDC we help you through your problems. We walk the path with you, in hopes to give you a better quality of life.

Our Behavioral health consultants offer one-on-one appointments for patients of all ages. Depression & Anxiety
Depression and anxiety make life difficult, just like any other chronic illness. Like diabetes patients need to maintain their blood sugar, people with depression and anxiety need to maintain their mental health.
We screen for these symptoms at all our patient visits, with the goal of maximizing your quality of life. Our staff is qualified to help provide you resources and tools to fight mental illness. From support groups to medication, our integrated behavioral health specialists treat patients on a person-by-person basis.
Our Behavioral Health team works with our Case Management team to connect you with any resource you may need. For example, we may refer you to one of our partners for additional support.

Flexible Payment Options
We accept most major insurances, including all PA Medicaid HMO's, DHS Medicaid, Medicare, and commercial plans. If you have no insurance, but still wish to visit a Provider, we can help! CHDC offers a health care discount program that is based on your family size and household income. Patients in our service area throughout Berks, Chester, and Montgomery counties who are eligible can receive discounts of up to 25%, 40%, 55%, 70% or 100% for services received at CHDC.
