Dear Valued Patient,

Thank you for allowing Community Health and Dental Care, Inc. to be your health care provider.

We wanted to make you aware of some upcoming changes that will affect your health insurance plan, Aetna Better Health. Starting in July, Aetna Better Health will be terminated as a Medicaid plan. We wanted you to know ahead of time so you can start thinking about which plan you will be switching to. Correspondence from the Department of Human Services was sent out about this transition in early April. You have been able to switch to a new payer since early May.  (The initial deadline of June 22, 2022 has been extended.)  You must select a new plan by August 16, 2022.*

CHDC accepts the following Medicaid plans (see the list below). Once you have made the change you can notify us by calling our office or sending us a secure message through the patient portal.

To change your plan:

• Go to PA Enrollment Services (enrollnow.net), or
• For regular Medicaid Enrollment services, call 1-800-440-3989 (TTY: 1-800-618-4225) and select Option 6 to speak with a representative. You can call Monday through Friday, 8 AM to 6 PM.
• For dual eligibility with Aetna Community Health Choice, Medicare/Medicaid Enrollment, call 1-844-824-3655

*Note, if you have not selected a new plan by August 16, 2022 one will be chosen for you by the Department of Human Services.*
