Healthy Heart Ambassador: Blood Pressure Self-Monitoring Program

Become a Healthy Heart Ambassador by joining the Blood Pressure Self-Monitoring Program! This free 4-month program includes a free blood pressure monitor, coaching twice a month, monthly nutrition seminars at CHDC, and weekly check-ins from a trained program facilitator. Monthly seminars will occur at the CHDC Shoppes at Coventry (formerly Coventry Mall) location on the 1st Thursday of each month March – June from 11:00AM – 12:00PM.

Participants will learn the right way to take their blood pressure and learn to measure and track their blood pressure at home. Participation Requirements:

• Must be 18 years old or older
• Must have a HBP diagnosis or on BP medication
• No cardiac events in last 6 months
• No atrial fibrillation or arrhythmias
• No risk of or diagnosed lymphedema
