Community Health and Dental Care (CHDC) is being noted as the best kept secret in local healthcare. Located at 11 Robinson St., Pottstown, CHDC opened its doors in October 2008 through the generous funding of the Pottstown Area Health and Wellness Foundation and various local organizations. During the first week, 39 patients were provided medical and dental services. Now, CHDC is caring for over 10,000 patients and counting! CHDC offers more than pediatric/adult primary and dental care as we provide case management, referral assistance, on-site Nutritionist, Nurse Navigators and overall dedicated staff to improve the quality of care for all ages. In addition, CHDC has extended evening and weekend hours and same day appointments to improve access for patients.
Operating as a non-profit, Federally Qualified Health Center (FQHC), CHDC’s mission is to identify gaps in health care services and ensure access to appropriate levels of care for all people in the service area regardless of their ability to pay. With that said, CHDC is able to offer health care discounts based on income and federal poverty levels. In addition, Medicaid, Medicare and most private insurances are also accepted and you can be a patient even if you do not have insurance.
Recently, Community Health and Dental Care was awarded a highly competitive grant from the U.S. Department of Health and Human Services. These funds will ensure that CHDC is able to carry out its mission of providing high-quality, affordable health care to everyone in this community.
Community Health and Dental Care is committed to making a difference in this community by promoting literacy through its “Reach out and Read” program which provides a free book to all pediatric patients at each wellness visit. In addition, CHDC participates in the Vaccines for Children (VFC) program that enables all qualifying pediatric patients to receive their immunizations at no charge. Also, a dental education program called “D.K. Destroyers” has been created for area elementary school students to learn the importance of good oral hygiene. Along with these initiatives, CHDC has been reaching out to the community by participating in the numerous events that have been held by PEAK (Pottstown Early Action for Kindergarten Readiness), Pottstown’s YMCA and YWCA, ACLAMO, Pottstown Family Center and Boyertown Area Community Wellness Council. In addition, CHDC is adding a dispensing pharmacy in late summer of 2012, available to all CHDC patients who can get their prescriptions on site and have the option to receive refills in the mail at a federal cost savings program for our patients!
Due to their ever increasing patient base, and the obligation to provide accessible health care, Community Health and Dental Care recently relocated their Administrative offices to Heritage Drive in Pottstown. This move created additional space for staff and patients at the Robinson Street location. CHDC’s highly qualified, professional staff is ensuring that everyone in this community, whether they are insured or not, has a place to go for high-quality, affordable medical and dental care.
